We ask that you please fill out the NEW PATIENT FORMS. In addition, the following forms are helpful to your provider:  INDIVIDUAL PATIENT, COUPLES or CHILD. They will help your provider to understand your history and current situation, but most importantly, not take up too much time in your first session gathering this much needed information. Please complete these and bring them with you on your first visit.

Payment and Insurance Verification

We are happy to assist you in verifying your eligibility for insurance coverage. Our staff will attempt to provide insurance verification over the phone. Call us to speak with our intake specialists and have the following information available:

  • Insurance Company name

  • Subscriber ID number and/or Policy Number

  • Insurance Company's phone number and any specific phone number for Behavioral Services

  • Prospective patient/client's name, address, phone number

  • Insured's date of birth

  • Patient/client's date of birth 

  • We accept insurance payments for treatment; however, we do not accept all insurance. Please refer to our list or call.

  • As with all medical services if there is a problem billing your insurance or you have lost coverage you will be financially responsible for the services.

The Counseling Center of New Smyrna Beach accepts Visa, Master Card, Discover, AMEX, check or cash for treatment payments. Please note, for all credit card payments there is a 3% surcharge.

Good Faith Estimate for Counseling Services

When you call to schedule we will tell you the cost per session for private pay rates up front. Please understand that the good faith estimate is per session not an accumulation of service dates. At present we do not offer any services that exceeds the minimum amount of $400.00 per session. However, the Government states that we must inform you of the following.

Your Right to a Good Faith Estimate

Upon request you have the right to receive a "Good Faith Estimate" explaining how much your medical care will cost.

What is a Good Faith Estimate?

A Good Faith Estimate (GFE) is an estimate of the reasonably anticipated charges for scheduled non-emergency items or services. This estimate is based on information known at the time the estimate was requested.

Who Does This Apply To?

This applies to you if you are:

  • Uninsured (you do not have health insurance coverage).

  • Self-pay (you have insurance but choose not to use it for specific services).

When Will You Receive Your Estimate?

  • Upon requesting if you schedule an appointment at least 3 business days in advance, you will receive a Good Faith Estimate no later than 1 business day after scheduling.

  • Upon requesting if you schedule an appointment at least 10 business days in advance, you will receive a Good Faith Estimate no later than 3 business days after scheduling.

  • Upon requesting if the service is unscheduled or requested by you, a Good Faith Estimate will be provided no later than 3 business days after the request.

What Information Will Your Good Faith Estimate Include?

If requested Your Good Faith Estimate will include:

  • A description of the counseling services you are expected to receive.

  • The estimated cost for each session.

  • The total estimated cost for all services per session.

  • The name and contact information of the Counseling Center of New Smyrna Beach.

  • Disclaimers about the estimate.

Important Considerations:

  • The Good Faith Estimate shows the costs of items and services that are reasonably expected for your health care needs for an item or service. The estimate does not include any unknown or unexpected costs that may arise during treatment.

  • You could be charged more if there are unforeseen circumstances or complications, or if additional services are needed that were not anticipated at the time the estimate was provided.

  • If you receive a bill that is more than $400.00 higher than your Good Faith Estimate, you have the right to dispute the bill.

Dispute Process:

For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises or call 1-800-985-3059.

If you believe you have been billed an amount at least $400.00 in excess of your Good Faith Estimate, you may submit a dispute resolution request to the U.S. Department of Health and Human Services (HHS) within 120 calendar days of the date on the bill.
