If you are wanting to set up a new patient appointment please give the office a call first at (386) 423-9161 and speak with one of our intake specialist. We try our best to find the Counselor or Provider that is the right fit for you. Once you have an appointment we will ask you to return here to complete the needed forms based on your appointment type. Paperwork, ID and copies of the front and back of your insurance card are due the week before your scheduled appointment. Thank you for understanding.
